Student Media Consent and Release Form* Throughout the school year, students may be highlighted in efforts to promote the activities and accomplishments of Integration Charter Schools (ICS) and each of its schools and students. For example, students and student work may be featured in materials to increase public awareness of our schools through print media, social media, radio, TV, the internet, DVDs, displays, brochures, newsletters and other types of media, including on the ICS website. 

To give ICS, its schools, collaborative partners, and each of their employees, representatives, agents, and authorized media organizations permission to videotape, photograph, interview, and/or record my child or my child’s school work for use in audio, video, film, and any other electronic, digital, and printed media both during and after my child’s enrollment at an ICS school;

To grant ICS, its schools, and its collaborative partners permission and the right to edit, use, and reuse any such products for non-profit purposes, including use in print, on the internet, and all other forms of media, including social media (I am fully aware that I will not receive monetary compensation for my child’s participation); and

To the use of my child’s first and last name by ICS in connection with the posting of my child’s work by an ICS employee on the ICS website or on any social media controlled by ICS.

I hereby release ICS, its schools, and its Trustees, employees, representatives, agents, and authorized media organizations from all claims, demands, and liabilities whatsoever in connection with any consent I have given above.
